- Download the booklet and set measurable goals for each factor that is contributing to high blood pressure eg reduce alcohol intake, reduce weight by healthy eating, use less salt when cooking, take prescribed medication.

- If you are struggling attaining goals in any sphere of your health care make an appointment with the appropriate member of the team to aid you in achieving goals eg. if you are having difficulty quitting smoking, make an appointment with a life coach.
